  43.251.  Report forms — how provided, contents — approval by superintendent. — 1. The Missouri division of highway safety shall prepare and upon request supply to police departments, sheriffs, and other appropriate agencies or individuals forms for written accident reports as required by sections 43.250 and 43.251. The written reports shall call for sufficiently detailed information to disclose, with reference to a vehicle accident, the cause, conditions then existing and the persons and vehicles involved.

  2. Every accident report required to be made in writing shall be made on the appropriate form approved by the superintendent of the Missouri state highway patrol and shall contain all the information required therein unless not available.
